Mississippi MHSAA Athletic Pre-Participation Physical

Mississippi high school athletes complete the MSMOC 62 Athletic Pre-Participation Form, posted by MHSAA as its physical form. Page 1 holds the family cardiac history, orthopedic and medical history and a waiver; page 2, completed by the physician only, records two blood pressure readings, a general medical, flexibility and orthopedic exam, and the screening result.

Who completes it

The examining physician completes page 2 and signs on the line marked M.D. The student and parent complete page 1 and sign the waiver.

Submitting and payment

How to submit it
The completed form is returned to the student's school; the form asks that it not be folded.
Who pays
The waiver says the exam is given without expectation of payment and cites Mississippi law on immunity for physicians who volunteer services to accredited school programs.

Good to know

  • The result is one of two boxes: no reason seen from this limited screening why the student cannot participate, or further evaluation needed as described.
  • Optional dental and vision exams appear at the bottom of page 2.
  • Family history asks about relatives under 50 with sudden death, hypertrophic cardiomyopathy, Marfan syndrome, long or short QT, Brugada syndrome and CPVT.
  • The current version is MSMOC 62, revised March 2022, posted by MHSAA as its physical form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
