NDHSAA Sports Physical Form (North Dakota PPE)

North Dakota students in NDHSAA-sanctioned sports must file a preparticipation health history screening and physical examination clearance with the school office every year. The packet follows the PPE 5th edition: history form, athletes with disabilities supplement, physical examination form and a Medical Eligibility Form. Only the eligibility page goes to the school; the medical facility keeps the rest.

Who completes it

The parent and athlete fill out the history form (and the disabilities supplement only if the athlete has special needs) before the visit; an MD, DO, PA or NP completes the physical examination and signs the Medical Eligibility Form. A parent signs the permission for emergency medical treatment on the eligibility page.

Submitting and payment

How to submit it
The family returns the signed Medical Eligibility Form to the school office before the student participates, while the history and physical examination pages stay in the medical facility's file.
Who pays
NDHSAA sets no fee for the exam and the form does not say who pays for it.

Good to know

  • The health history screening and physical examination are valid for one school year, and an exam completed on or after April 15 counts for the following school year.
  • The eligibility page offers five outcomes, from medically eligible for all sports without restriction to not medically eligible for any sports, with space for recommendations.
  • The provider's statement notes that exam findings are on record in the office and can be made available to the school at the parents' request.
  • A shared emergency information block on the eligibility page lists allergies, medications, other information and emergency contacts for school staff.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Who can sign the NDHSAA physical form?

Under the NDHSAA Constitution and By-Laws, physical evaluations may be done by a medical doctor, doctor of osteopathy, physician assistant or nurse practitioner. The signature lines on both the physical examination page and the Medical Eligibility Form list MD, DO, NP or PA after the professional's name.

How long is an NDHSAA sports physical good for?

One school year. NDHSAA says the health history screening and physical examination are valid for a single school year, and an exam completed on or after April 15 is valid for participation in the following school year. Students file a new clearance every year before participating.

Which page of the North Dakota physical goes to the school?

Only the Medical Eligibility Form. NDHSAA's instructions say the medical facility should keep the history form, the athletes with disabilities supplement and the physical examination form, and that the eligibility form is the only page returned to the school office.
