New Jersey PPE Form: Sports Physical (NJDOE)

New Jersey law (N.J.S.A. 18A:40-41.7) requires a preparticipation physical before a public-school student joins a school sport, and the Department of Education's 2024 form is the version NJSIAA posts. The five-page packet adds a PHQ-4 mental health screen to the PPE history, and only the Medical Eligibility Form is submitted to the school.

Who completes it

The student and parent complete and sign the history form before the appointment. A licensed physician, advanced practice nurse or physician assistant who has completed the NJDOE Student-Athlete Cardiac Assessment Professional Development module performs the exam and signs the eligibility form, certifying that training.

Submitting and payment

How to submit it
The family gives the Medical Eligibility Form to the school, which keeps it with the student's school health record; the history and physical exam stay with the provider (medical home) and should not be shared with the school.
Who pays
The form sets no fee and does not say who pays for the exam.

Good to know

  • The eligibility page has a separate signed certification that the provider completed the Cardiac Assessment Professional Development Module developed by the New Jersey Department of Education.
  • The history form includes PHQ-4 anxiety and depression questions, sex assigned at birth, gender identity and COVID-19 vaccination questions.
  • NJDOE's Health History Update Form, completed by the parent in English or Spanish, is used when the physical is more than 60 days old.
  • The form belongs to the New Jersey Department of Education; NJSIAA posts it but states that it is not an NJSIAA form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can perform a New Jersey sports physical?

A licensed physician, advanced practice nurse or physician assistant who has completed the Student-Athlete Cardiac Assessment Professional Development module hosted by the New Jersey Department of Education. The provider signs a certification of that training on the Medical Eligibility Form, next to an optional office stamp.

Is the New Jersey PPE form available in Spanish?

Yes. NJSIAA posts a Spanish version of the 2024 NJDOE preparticipation physical evaluation form alongside the English one, and the Health History Update Form and sudden cardiac death pamphlet sign-off are also offered in both languages.

What does the school receive from the New Jersey PPE?

Only the Medical Eligibility Form, which records the eligibility decision, recommendations, the provider's signature and cardiac module certification, and shared health information such as allergies, medications and emergency contacts. The history and exam pages are kept by the medical home and are not shared with schools.
