Alaska ASAA Student Health Review/Exam Physical Form

ASAA requires a statement on file with the superintendent or principal that the student had a history and physical exam within the prior 18 months before any practice or competition, including rifle and cheer. This two-page form provides it: the parent completes the health history on page 1, and the examiner records the physical exam and clearance on page 2.

Who completes it

A physician, physician assistant, advanced nurse practitioner or doctor of chiropractic completes and signs page 2, circling the credential; ASAA also accepts a military medical examiner. The parent or guardian completes page 1, and the student and parent sign it.

Submitting and payment

How to submit it
The completed form goes to the school, not to ASAA, and must be on file with the superintendent or principal before the student practices or competes.
Who pays
ASAA sets no fee for the exam and does not say who pays.

Good to know

  • Alaska accepts a history and physical from within the prior 18 months, a longer window than the annual exam many state associations require.
  • The page 1 history asks for immunization dates, including the last tetanus shot, measles shot and TB skin test, along with cardiac symptoms, concussions and menstrual history.
  • Clearance options are cleared, cleared with recommendations for further evaluation or treatment, or not cleared pending evaluation, for any sports or for certain sports, with a reason.
  • ASAA pairs this form with its Authorization to Release Medical Information form, which the student and parent sign; submitting both satisfies the requirement.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long is an Alaska school sports physical valid?

ASAA requires a history and physical examination within the prior 18 months. The statement certifying it must be on file with the superintendent or principal before the student practices or competes in any athletic event, and parental permission is required before each year of participation.

Who can sign the ASAA Student Health Review/Exam?

Page 2 is completed by a physician, physician assistant, advanced nurse practitioner or doctor of chiropractic, who circles MD, DO, PA, ANP or DC by the signature. ASAA's pre-participation page also lists a military medical examiner as an accepted signer.

Where does the ASAA physical form go?

The form states it is sent to the school and not to ASAA. The school keeps it on file with the superintendent or principal, together with the Authorization to Release Medical Information form signed by the student and parent.
