SCHSL Physical Form (South Carolina Sports PPE)

South Carolina High School League athletes need this three-page preparticipation physical, updated January 7, 2025, before competing. Based on the PPE 5th edition, it puts the examination and medical eligibility statement on the first page, the athlete's medical history on the second and a parent's permission and acknowledgment of risk on the third.

Who completes it

The athlete, with a parent if under 18, completes and signs the history page before the appointment. An MD, DO, NP or PA records the exam, marks an eligibility decision and signs, and the parent and athlete sign the permission page.

Submitting and payment

How to submit it
The completed form is returned to the athlete's school.
Who pays
SCHSL sets no fee and the form does not say who pays for the physical.

Good to know

  • The exam page lists skin findings to check, including herpes simplex, lesions suggestive of MRSA and tinea corporis, plus double-leg and single-leg squat and box or step drop tests.
  • Eligibility options run from medically eligible for all sports without restriction to not medically eligible for any sports, with space to name certain sports and recommendations.
  • The provider's statement says eligibility may be rescinded if conditions arise after clearance, until the problem is resolved and explained to the athlete and parents.
  • The parent permission page notes the evaluation is a screening, not a substitute for regular health care, and that the data may be used for research.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the SCHSL physical form?

The signature line on the examination page is labeled MD, DO, NP or PA. The health care professional prints their name, address and phone, dates the form and signs below the statement that they examined the student and completed the preparticipation physical evaluation.

What does the parent sign on the SCHSL physical?

The parent or guardian signs the history page with the athlete and a separate Parent's Permission and Acknowledgement of Risk page. That page grants permission for participation and for treatment of conditions arising during events, and gives nurses, athletic trainers, coaches and physicians access to necessary medical information.

Does SCHSL have a concussion clearance form for doctors?

No. SCHSL publishes return-to-play guidelines for concussion but no physician clearance form, so this physical form is the SCHSL document a clinician signs for routine participation. It asks athletes whether they have had a concussion or head injury.
