Massachusetts Post Sports-Related Head Injury Clearance

Massachusetts regulations (105 CMR 201) require medical clearance before a student removed from an extracurricular athletic activity for a head injury or suspected concussion returns. DPH's form records it in two stages: clearance to begin a gradual return-to-play protocol, and certification that the student has completed it and is cleared for full activity without restriction.

Who completes it

A physician, licensed athletic trainer, physician assistant, nurse practitioner or neuropsychologist completes and signs it, attesting to DPH-approved or equivalent training; non-physicians must work in consultation with a licensed physician, who is named on the form.

Submitting and payment

How to submit it
The student returns the completed form to the school's athletic director, athletic trainer or school nurse.
Who pays
DPH sets no fee for the clearance and does not say who pays.

Good to know

  • Do not clear a student to begin the protocol while they still need concussion-related academic accommodations; they must be back in class full time.
  • To clear for full activity, confirm verbally that stages 1 to 4 of the six-stage protocol were completed without recurring symptoms.
  • The form is not valid without the training attestation, and the type of training (CDC online clinician training, MDPH-approved training or other) must be completed.
  • DPH posts the form in English, Spanish and Portuguese, as PDF and Word files.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
