VHSL Physical Form (Virginia Sports PPE)

Virginia High School League athletes must give the principal a fully completed Athletic Participation/Parental Consent/Evaluation Form before any tryout or practice. The six-page form, revised April 2025, combines eligibility rules, parent consent and emergency permission, the PPE Medical Eligibility Form, the medical history and the physical exam page.

Who completes it

The student and parent complete Part I (participation), Part II (risk acknowledgment and insurance), Part III (emergency permission) and the history. The examining MD, DO, NP or PA completes the physical examination and signs the Medical Eligibility Form on page 3.

Submitting and payment

How to submit it
Pages 1 to 3 must be submitted to the school; the history and physical examination pages go into the athlete's medical file.
Who pays
VHSL sets no fee and the form does not say who pays for the exam.

Good to know

  • The form expires 14 months from the date of the practitioner's signature on page 3.
  • The exam page records whether a COVID-19 vaccine was previously received or given at the visit, with dose numbers.
  • Parents consent to treatment and to providers sharing participation-relevant information with coaches and school staff, and list emergency medications such as an inhaler or epinephrine pen.
  • VHSL notes that the pre-participation examination is not a substitute for a thorough annual examination by the student's primary care physician.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
