WIAA Wisconsin Wrestling Skin Lesion Medical Release

Wisconsin's WIAA version of the wrestling skin lesion release documents a wrestler's diagnosis, contagious status and treatment so officials can let the wrestler compete. It adds contagious and non-contagious checkboxes, a license number field and a note telling officials how long the form stays valid.

Who completes it

An MD, DO, PA or APNP completes the whole form legibly, including diagnosis, contagious status, lesion diagram, medication name, dose, frequency and days prescribed, treatment start, expiration and return dates, license number and signature.

Submitting and payment

How to submit it
The wrestler's coach presents the completed release to meet officials at weigh-in for the dual meet or tournament.
Who pays
WIAA sets no fee and the form does not say who pays for the visit.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
