Arkansas AAA Physical Exam Form (PPE Evaluation)

The Arkansas Activities Association posts this one-page physical examination form among its student medicine forms for interscholastic athletes. It follows the 2019 preparticipation physical evaluation exam page: vitals, medical and musculoskeletal findings, functional testing and a clearance decision. It pairs with the AAA history form, which the student and family complete in English or Spanish before the visit.

Who completes it

The examining physician, APN or PA completes the exam and clearance and signs. The student and parent complete the separate Physical Exam Form - History.

Submitting and payment

How to submit it
The completed form is returned to the student's school.
Who pays
AAA sets no fee for the exam and does not say who pays.

Good to know

  • Clearance choices are cleared without restriction, cleared with recommendations for further evaluation or treatment, or not cleared, pending further evaluation, for any sports or certain sports, with a reason.
  • The heart section reminds examiners to consider ECG, echocardiography or cardiology referral for abnormal cardiac history or exam findings.
  • Skin is checked for herpes simplex, MRSA and tinea corporis, and the functional exam lists double-leg and single-leg squats and a box or step drop test.
  • AAA links the forms from its Sports Medicine Info page, together with its graduated return-to-play protocol and sport-specific return-to-play protocols.

Fill it with Vero

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Who can sign the Arkansas AAA physical exam form?

The signature line reads physician, APN or PA, so an advanced practice nurse or physician assistant can complete and sign the exam as well as a physician. The provider prints name, address, phone and date of exam.

Is the AAA physical form available in Spanish?

AAA posts the history portion, Physical Exam Form - History, in English and Spanish. The evaluation page that the provider completes is posted in English, so a Spanish-speaking family can complete the history in Spanish and the provider completes the English exam page.

Can a clearance on the AAA form be withdrawn?

Yes. The attestation states that if conditions arise after the athlete has been cleared, the physician may rescind the clearance until the problem is resolved and the potential consequences are completely explained to the athlete and parents or guardians.
