WIAA Washington Sports Physical Forms (PPE)

The Washington Interscholastic Activities Association recommends the national PPE forms for interscholastic athletics and posts the Physical Examination Form and Medical Eligibility Form on its forms page. The eligibility form records the clearance decision and shared emergency information, and it is the only page that goes to the school.

Who completes it

The athlete and parent complete the AAP medical history form (English or Spanish) before the visit. The examining clinician completes the physical examination form and signs the Medical Eligibility Form; both signature lines list MD, DO, ARNP, PA and ND.

Submitting and payment

How to submit it
The Medical Eligibility Form goes to the school or sports organization; the physical examination form is placed in the athlete's medical file and is not shared with schools.
Who pays
WIAA sets no fee and the forms do not say who pays for the exam.

Good to know

  • The exam form notes that athletes with a current PPE on file, per state and local guidance, should not need to complete another examination.
  • The signature lines include ND, so naturopathic doctors appear alongside MDs, DOs, ARNPs and PAs on the Washington version of the forms.
  • Physician reminders cover stress, mood, safety at home, tobacco, alcohol, drugs, anabolic steroids, supplements, seat belts, helmets and condoms.
  • WIAA's forms page posts no concussion clearance template or wrestling skin form alongside these physical forms.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the WIAA physical form in Washington?
