Idaho IHSAA Pre-Participation Physical and Consent

The Idaho High School Activities Association requires a history and preparticipation physical before a student-athlete's first practice in 9th grade and again in 11th grade. This three-part form covers it: the family completes the Part 1 history, the provider completes the Part 2 physical exam and clearance, and the parent and student sign the Part 3 consent to treat.

Who completes it

Under IHSAA rule 13-4, only a licensed physician, physician assistant or nurse practitioner may complete Part 2, the physical exam. The student and parent complete the Part 1 history and sign the Part 3 consent.

Submitting and payment

How to submit it
The completed form goes to the student's school; the school or district has final authority to accept it.
Who pays
The form states the exam is the student-athlete's responsibility; for an uncompensated exam in a school health program, Idaho Code 39-7703 gives the provider civil immunity.

Good to know

  • The exam must be completed after May 1 of the 8th and 10th grade years. Idaho uses an interim questionnaire in the years between full physicals.
  • Part 2 includes a biological sex determination based on reproductive anatomy, genetic makeup or endogenous testosterone levels, with a yes or no box.
  • The Part 3 consent meets Idaho Code 32-1015 parental consent requirements and covers travel and health care for conditions resulting from athletic participation.
  • The Spanish version, updated July 28, 2026, includes an English copy of Part 2 for the examining provider.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
