Arizona AIA Form 15.7-B Physical Examination

Arizona Interscholastic Association Bylaw 15.7.1 bars a student from practice or competition until a preparticipation physical exam record is on file with the principal or designee. Form 15.7-B is the examining provider's page of that record: vitals with up to three blood pressure readings, a medical and musculoskeletal exam, and a clearance decision with notes.

Who completes it

The examining provider completes and signs 15.7-B and circles a credential: MD, DO, ND, NP, PA-C or CCSP. The parent and student complete Forms 15.7-A (history), 15.7-C (acknowledgement) and 15.7-D (consent to treat).

Submitting and payment

How to submit it
The completed form goes to the student's school, where the record must be on file with the principal or designee before the first practice.
Who pays
AIA sets no fee for the physical and does not say who pays.

  • The principal or designee may require a student to be re-examined if they consider it advisable.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When can a physical for next school year be done in Arizona?

AIA Bylaw 15.7.1 says the physical examination for the following school year must be given on or after March 1. An exam earlier than that does not count for the next year, and the record must be on file before the student practices or competes.

Who can sign AIA Form 15.7-B?

An M.D., D.O., N.D. or N.M.D., N.P., PA-C, or a certified chiropractic sports physician (CCSP). The examination must state that the provider found no medical reason to disqualify the student, or list restrictions or the reason the student is not cleared.

What other forms go with 15.7-B?

Form 15.7-A is the history the parent and student complete, 15.7-C is an acknowledgement and 15.7-D is consent to treat, both signed by the parent and student. AIA also posts a combined file with all four forms.
