Medical cannabis certifications and registry portals for clinicians

How clinicians certify patients for each state's medical cannabis program: paper certification forms, registry portals, certifications for minors and renewals.

Each state medical cannabis program has its own clinician step: a certification, recommendation or written statement that the patient qualifies. Many of the 62 entries on this page are not paper forms at all. The certification is entered in a state registry, such as Utah's Electronic Verification System, New York's MCDMS, the Illinois provider portal, Pennsylvania's Practitioner Registry and Ohio's patient and caregiver registry. Other states still use a PDF the patient submits, such as the Arkansas Physician Written Certification, Michigan's MMP 3501 and Washington's DOH 623-123.

Rules on who may certify differ. Florida's qualified physicians take a 2-hour course, Pennsylvania and West Virginia require a 4-hour course, and Ohio physicians need a Certificate to Recommend. Alabama, Colorado and Florida require an in-person exam, while Pennsylvania allows telemedicine certifications. Minors often need a second physician, as on Michigan's MMP 3500-4 and Oklahoma's minor recommendation form. The patient pays the state registry fee. Vero Documents fills PDF forms from the visit note and the chart; the clinician reviews and signs.

Frequently asked questions

Can a nurse practitioner certify a patient for medical cannabis?

In many states, yes. NPs or APRNs can certify in Connecticut, Illinois, Maryland, Massachusetts, Minnesota, Missouri, New Jersey, New York, Utah, Virginia and Washington, among others. Some programs are physician-only: Alabama, Arkansas, Florida, Georgia, Michigan, Montana, Ohio and Pennsylvania limit certification to MDs and DOs, and Oklahoma also accepts podiatric physicians. The list below shows who completes each state's certification.

How do I certify a patient for medical marijuana in Pennsylvania?

Pennsylvania certification is portal-only. The MD or DO first completes a 4-hour approved training course and is approved by the Department of Health. Then, in the Practitioner Registry, choose Patient Certifications and Create New Certification, list up to 10 serious medical conditions, and record the certification number in the chart. Telemedicine certifications are allowed. The patient uses the certification to get a medical marijuana ID card.

Does a minor need two physicians for a medical cannabis card?

In several states, yes. Michigan's MMP 3500-4 packet needs certifications from two Michigan MDs or DOs. Oklahoma's minor form needs two physicians, with the recommendations dated within 30 days of each other. Illinois needs a recommending and a reviewing certification from different professionals for non-terminal minors, Arizona adds a reviewing physician certification, and Montana's PSDMC-Minor needs a second physician unless the first is an oncologist, neurologist or epileptologist.

What makes a Washington medical cannabis authorization valid?

Only the official DOH 623-123 form, printed on tamper-resistant paper bearing the RCW 69.51A.030 logo, is valid. A Washington-licensed MD, DO, PA, ARNP or naturopath signs and seals it and gives it to the patient, who takes it to an endorsed retailer for registry entry. The first authorization must be in person. It lasts up to 1 year for adults and up to 6 months for minors.
