Illinois Medical Cannabis Certification (Adult MCPP)

Illinois adults applying for a medical cannabis registry card need an electronic certificate from a health care professional. The professional creates it in the IDPH Medical Cannabis Provider Portal, choosing the Adult MCPP category, entering the patient's information and debilitating condition, and flagging terminal illness or a lifelong condition. The patient then adds the certificate to their registry application.

Who completes it

An Illinois-licensed physician, APRN or PA with a controlled substances license and a bona fide relationship with the patient, registered in the IDPH provider portal, completes the whole electronic certificate. The patient completes the registry application separately.

Submitting and payment

How to submit it
Create it in the IDPH Medical Cannabis Provider Portal through ILogin with New Certificate; it stays Eligible until the patient adds it to their application, when it becomes Active.

Good to know

  • Answer the terminal illness question on every adult certificate; terminal patients get expedited processing, and the condition is explained under Other.
  • For a lifelong condition, enter a Lifelong Condition Explanation on the certificate so the patient's registration reflects it.
  • Providers can amend, renew, cancel or revoke certificates in the same portal, and IDPH posts separate instructions for each action.
  • IDPH lists more than 50 qualifying conditions, from autism, cancer and chronic pain to PTSD, migraines, osteoarthritis, traumatic brain injury and ulcerative colitis.

Frequently asked questions

What does Eligible mean on an Illinois medical cannabis certificate?

A new certificate starts with the status Eligible in the IDPH Provider Portal. It stays Eligible until the patient adds the certification to their registry application, and then the status changes to Active. The provider can view each certificate's details from the portal dashboard.

Which conditions qualify for the Illinois Medical Cannabis Patient Program?

IDPH lists more than 50, including autism, agitation of Alzheimer's disease, HIV/AIDS, ALS, cancer, chronic pain, Crohn's disease, Ehlers-Danlos syndrome, glaucoma, migraines, multiple sclerosis, neuropathy, osteoarthritis, Parkinson's disease, PTSD, seizures, sickle cell disease, traumatic brain injury and ulcerative colitis.

Can APRNs and PAs certify Illinois medical cannabis patients?

Yes. Illinois-licensed APRNs and physician assistants, as well as physicians, may create adult certificates if they hold a controlled substances license, have a bona fide relationship with the patient and are registered in the IDPH provider portal. The Opioid Alternative Patient Program differs: its certification must come from a physician.
