Utah Medical Cannabis Card Certification (EVS)

Utah medical cannabis cards start with a certification from a recommending medical provider (RMP) in the Electronic Verification System (EVS). After the patient starts a card application, the RMP or a proxy enters the qualifying condition, card length and optional dosing guidelines; the patient then pays and receives the card. Cards are issued for one year unless the provider chooses a shorter period.

Who completes it

A Utah-licensed MD, DO, APRN, PA or podiatrist registered as an RMP, with 4 hours of medical cannabis continuing education in the past 24 months, completes the certification section; staff proxies can review, edit and submit it for the RMP. The patient completes the rest of the application.

Submitting and payment

How to submit it
Enter the certification in the patient's EVS application after they start it (a Utah ID login is required), or send the paper recommendation form to a medical cannabis pharmacy instead.
Who pays
The patient pays the card fee in EVS; RMPs may bill insurance for the appointment, and patients billed to insurance do not count toward the RMP's patient limit.

Good to know

  • The first card appointment must be in person, except for terminal illness, hospice or a facility resident who cannot attend; follow-up and renewal visits can be by telehealth.
  • RMPs may recommend for up to 1.5% of Utah medical cannabis patients, currently 1,759 recommendations, with the limit updated January 1, 2027.
  • Dosing guidelines can set product types and monthly amounts in EVS, or you can defer dosing to a pharmacist, who will contact you with any suggested changes.
  • Patients under 21 or without a qualifying condition need a Compassionate Use Board petition, created in EVS after the certification is entered.
