Utah Compassionate Use Board (CUB) Petition

Utah's Compassionate Use Board reviews medical cannabis card applications for patients under 21 and for patients who do not have a qualifying condition but may still benefit. The recommending provider who certified the patient submits a petition covering the patient's age, medical history, and treatments tried or in use. The Board will not review an application without both a certification and a petition.

Who completes it

The recommending medical provider who certified the patient completes the whole petition. The patient pays the card fee in EVS, after which the application status changes to Awaiting CUB Review.

Submitting and payment

How to submit it
Create and submit the petition in EVS after entering the certification; if the certification went through a pharmacy on the paper form, use the REDCap survey link emailed to you after the patient pays.
Who pays
The patient pays the card fee in EVS before the petition moves to Board review.

Good to know

  • Decisions are emailed within 90 days of payment (EVS) or of petition submission (REDCap). If the Board needs more information, it sets a return date and inactivates petitions that miss it.
  • Submission deadlines are periodic: petitions in by 5 p.m. on October 9, 2026 were scheduled for decisions by October 30, 2026.
  • Board meetings are private and confidential, even to the patient's medical provider; the Board is made up of seven medical providers from different specialties.
  • The Center for Medical Cannabis publishes a CUB petition guide, a medical condition guide and a REDCap petition guide for providers.
