NY Medical Cannabis Patient Certification (MCDMS)

New York practitioners certify patients for medical cannabis electronically, in the Medical Cannabis Data Management System (MCDMS) Provider Portal reached through the Health Commerce System. The certification states that the patient is likely to receive therapeutic or palliative benefit; it is printed for the patient, who then registers for a registry ID. Since the 2021 Marihuana Regulation and Taxation Act, no fixed list of qualifying conditions applies.

Who completes it

A New York-licensed practitioner with DEA registration, such as a physician, NP, PA, dentist, podiatrist or midwife, who has completed an approved course of at least two hours completes the whole certification. The patient registers afterward, and a designated caregiver is registered automatically.

Submitting and payment

How to submit it
Issue the certification in the MCDMS Provider Portal, which you add to My Applications in the Health Commerce System on first use, and print it for the patient, who then completes registration.
Who pays
The Office of Cannabis Management states that office visits for patient evaluation and certification are Medicaid reimbursable services, though medical cannabis products are not a covered Medicaid benefit.

Good to know

  • Approved online courses run three to six hours and cover pharmacology, contraindications, dosing, routes of administration, drug interactions, and abuse and dependence; at least two hours are required.
  • For patients under 18 or unable to consent, a caregiver over 18 is designated; that caregiver is auto-registered at certification and receives a registry ID document.
  • OCM publishes MCDMS how-to guides for issuing, editing, extending, canceling and printing certifications, plus a Practitioner Guide to Patient Certifications.
  • Report patient adverse events to the Office within five business days, and serious adverse events within one business day of becoming aware of them.

Frequently asked questions

Is there a list of qualifying conditions for medical cannabis in New York?

No fixed list applies. Since the 2021 Marihuana Regulation and Taxation Act, the practitioner decides whether a condition is likely to benefit from medical cannabis. The Office of Cannabis Management still lists common conditions for reference and publishes guidance on topics such as PTSD and the treatment of opioid use disorder.

How do I get access to MCDMS to certify patients?

Log in to the Health Commerce System, open All Applications and add the MCDMS Provider Portal to My Applications; returning users open it from My Applications. You need a New York practitioner license, DEA registration and a completed course of at least two hours before certifying patients.
