Washington Medical Cannabis Authorization (DOH 623-123)

Washington practitioners authorize medical cannabis on this one-page state form, which states that the patient may benefit from medical use and has a qualifying condition under RCW 69.51A.010(24). It is not a prescription. The authorization is the basis for entering the patient in the Medical Cannabis Registry and issuing a recognition card at an endorsed retailer, and it records compassionate-care renewal eligibility and any additional plants.

Who completes it

A Washington-licensed MD, DO, PA, ARNP or naturopath completes Sections II and III (items 7 to 13), then signs and seals the form. Section I holds the patient's and any designated provider's information, and an adult patient signs item 6 to name a designated provider.

Submitting and payment

How to submit it
Print it on vendor-supplied tamper-resistant paper bearing the RCW 69.51A.030 logo, sign and seal it, and give it to the patient, who takes it to an endorsed retailer for registry entry.

Good to know

  • The initial authorization must follow an in-person physical exam; renewals may use telemedicine only if you document that an in-person exam would likely cause the patient severe hardship.
  • Adult authorizations are valid for up to one year from issue, and minors' for up to six months; patients under 18 must have a parent or guardian as designated provider.
  • Patients may grow four plants, or six if entered in the registry; item 11 lets you authorize 7 to 15 plants when medical needs exceed that.
  • A separate, identical form is required for the designated provider. DOH's Practice Guidelines (DOH 631-053) call for at least three hours of continuing education.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can I print Washington's medical cannabis authorization on plain paper?

No. Authorizations must be printed on vendor-provided tamper-resistant paper with the RCW 69.51A.030 logo in the lower right corner. DOH does not keep a list of paper vendors, so practices ask their current vendor or find one themselves. DOH 608-048 explains what retailers check for a valid authorization.

How long is a Washington medical cannabis authorization valid?

Up to one year from the issue date for adults and up to six months for minor patients. Item 12 records the issue date and the date by which it must be renewed. Renewal requires an in-person physical exam unless you document severe hardship, which allows telemedicine.

What changed with Washington's Medical Cannabis Registry in 2025?

In July 2025, DOH replaced the Airlift system with the Medical Cannabis Registry. Practitioners can create an optional registry account to manage patient authorizations, and DOH posts FAQs and training guides for it. The form itself, revised December 2025, is still printed, signed and given to the patient.
