Ohio Medical Marijuana Recommendation (Registry)

Ohio has no paper recommendation form: a physician holding a Certificate to Recommend registers the patient, and any caregiver, in the Ohio Medical Marijuana Registry and records the recommendation for a qualifying condition. The patient then activates the registration and pays. Recommendations are renewed annually, and the Department of Commerce's Division of Cannabis Control has run the program since 2024.

Who completes it

An Ohio-licensed MD or DO with an active Certificate to Recommend from the State Medical Board of Ohio, or a delegate, completes the electronic registration and recommendation. The patient completes activation and payment.

Submitting and payment

How to submit it
Enter the patient, and any caregiver, in the Ohio Medical Marijuana Registry under your Certificate to Recommend; the patient then activates the registration and pays.
Who pays
The patient pays when activating the registration, and the registry has a separate process for registering indigent or veteran status.

Good to know

  • The Certificate to Recommend is issued by the State Medical Board of Ohio under OAC 4731-32 and must be active before you can register any patient.
  • The physician registers any caregiver in the registry along with the patient, so the caregiver's details are needed at the time of registration.
  • The program moved from the Board of Pharmacy and the Commerce medical marijuana control program to the Division of Cannabis Control in 2024.
