Arkansas Medical Marijuana Physician Certification

The Arkansas Department of Health (ADH) needs this certification from an Arkansas MD or DO before it issues a medical marijuana registry ID card. After assessing the patient's history and current condition, the physician checks one or more of 18 printed qualifying conditions, marks whether the patient is under 18 or physically disabled, and chooses a card length of 12 months or less.

Who completes it

An Arkansas-licensed MD or DO with a DEA registration completes the whole page and signs; no cannabis training is required. For a minor, the parent, legal guardian or custodian must be present and signs the consent at the bottom.

Submitting and payment

How to submit it
Give it to the patient, who submits it to ADH online or by mail with the patient application and fee within 30 days of the physician's signature; paper submissions need original signatures.
Who pays
The patient pays ADH's nonrefundable registry fee; the state does not regulate what the physician charges for the certification.

Good to know

  • Only the printed conditions may be checked. ADH rejects handwritten conditions and will not accept a letter or other certification in place of its form.
  • A new certification is needed for every renewal. Under Act 1112 of 2021, physician re-certification assessments may be done by telehealth.
  • Mark under 18 or physically disabled when a caregiver is needed; ADH cannot process a caregiver application unless one of those boxes is checked.
  • ADH returns incomplete certifications to the patient, and any correction must be initialed by the physician. ADH may contact the physician to verify the signature.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long is an Arkansas physician certification valid?

ADH must receive it, with the application and payment, within 30 days of the physician's signature; after that the patient needs a new one. The resulting registry card lasts up to 12 months, or a shorter period the physician enters in the Issue Registry Card For field.

Do Arkansas physicians have to sign medical marijuana certifications?

No. ADH states that physicians are not required to complete the certification for a patient. A physician who agrees must hold an unrestricted Arkansas MD or DO license and a DEA registration, complete every section and sign. No medical marijuana training is required.

Can an Arkansas certification be done by telehealth?

For renewals, yes. A footnote on the form says that under Act 1112 of 2021, physician written re-certification assessments may be done via telehealth. A new certification form is still required each time the patient renews the registry card.
