Illinois Minor Medical Cannabis Certification

Non-terminal Illinois patients under 18 in the Medical Cannabis Patient Program need two electronic certificates from two different health care professionals: one Recommending and one Reviewing. Each professional creates their certificate in the IDPH Provider Portal under the Minor MCPP category, and the qualifying conditions on both must be identical. The parent or guardian then links both certificates to the minor's application.

Who completes it

Two different Illinois health care professionals registered in the provider portal each complete a whole certificate, one selecting Recommending and the other Reviewing as the certification type. The parent or guardian completes the minor's application.

Submitting and payment

How to submit it
Each professional creates a certificate in the IDPH Medical Cannabis Provider Portal with New Certificate and the Minor MCPP category; linking them to the minor's application turns each from Eligible to Active.

Good to know

  • The Recommending and Reviewing certificates must list identical qualifying medical conditions, chosen from IDPH's debilitating conditions list.
  • For a terminally ill minor, check Other and enter an Other Debilitating Medical Condition Explanation; for a lifelong condition, enter a Lifelong Condition Explanation.
  • The two-certificate rule applies to non-terminal minors, and IDPH posts a separate Minor Qualifying Patient Application for families to complete.

Frequently asked questions

Does an Illinois minor need two medical cannabis certifications?

Yes, unless the minor is terminally ill. IDPH requires a Recommending and a Reviewing certification from two different health care professionals for a non-terminal minor, and the qualifying medical conditions on both must match. Each professional creates their own certificate in the IDPH Provider Portal.

How are the certificates linked to an Illinois minor's application?

Each certificate starts as Eligible in the provider portal. The parent or guardian links both certificates to the minor's application, at which point each certificate's status changes to Active. The professionals can view their certificates and details from the portal dashboard.
