NH Therapeutic Cannabis Written Certification

New Hampshire's Therapeutic Cannabis Program requires this certification for every initial or renewal registry ID card. The provider attests to a provider-patient relationship built on a full assessment and certifies a qualifying condition: a listed diagnosis with a qualifying symptom (Box A) or a stand-alone condition (Box B). The patient mails it with the Patient Application, fee and proof of residency.

Who completes it

A New Hampshire physician, APRN or PA, another NH prescriber primarily responsible for the condition (dentist, podiatrist, optometrist, naturopathic doctor), or a Maine, Massachusetts or Vermont physician, APRN or PA primarily responsible for it completes every field. All need active DEA registration.

Submitting and payment

How to submit it
Give the signed form to the patient rather than sending it to the Program; it is accepted for up to 6 months from the provider's signature, and the Program notifies the provider in writing of its decision.
Who pays
The patient pays the Program's application fee when submitting the Patient Application.

Good to know

  • Initial certification requires an in-person physical exam; telemedicine is allowed only for recertification by the same provider, and the form requires checking which applies.
  • Box B stand-alone conditions include generalized anxiety disorder, moderate or severe PTSD, chronic pain, autism, and opioid use disorder, which needs a provider board-certified in addiction medicine or addiction psychiatry.
  • The card is valid for 12 months by default; the provider may instead choose 3, 6, 18, 24 or 36 months, the maximum. Incomplete forms are returned to the provider.
  • Patients under 18 need two certifying providers, one a pediatric care provider. The provider also certifies counseling on pregnancy and breastfeeding risks, and on adolescent use for patients 25 or younger.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign a New Hampshire cannabis written certification?

New Hampshire physicians (MD/DO), APRNs and PAs with active DEA registration. Dentists, podiatrists, optometrists and naturopathic doctors in New Hampshire may certify for a condition they are primarily responsible for, as may physicians, APRNs and PAs from Maine, Massachusetts or Vermont who are primarily responsible for the patient's condition.

How long is a New Hampshire written certification valid?

The Program accepts a certification with an application for up to 6 months after the provider signs it. The registry card then runs 12 months by default, or 3 to 36 months as the provider selects. The certifying provider can lengthen a shorter certification with the Written Certification Extension, keeping the total within 3 years.

Can the cannabis certification exam be done by telemedicine in New Hampshire?

Not for an initial certification. Under He-C 401.06(b)(4), the full assessment includes an in-person physical examination, a medical and prescription history, review of tests, a documented diagnosis and a treatment plan. Telemedicine is allowed when the same provider recertifies the patient, and the form asks which applies.
