Iowa 470-0189 Medical Cannabis Certification

Iowa residents need this certification from a health care practitioner to get a medical cannabis registration card from the Bureau of Cannabis Regulation. The practitioner checks the qualifying debilitating conditions, initials six statements on the relationship, primary care role, medical judgment, patient education, annual review and program rules, and signs under penalty of perjury.

Who completes it

An Iowa-licensed MD or DO, a PA or ARNP who is the patient's primary care provider, or a podiatrist completes pages 2 to 4. The caregiver designation on the last page is completed only when a caregiver is designated, with the practitioner signing the designation.

Submitting and payment

How to submit it
Give it to the patient, who uploads it with the online patient application, ID and fee so the bureau receives it within 60 days of the practitioner's signature; HHS reviews and responds within seven days.
Who pays
The patient pays a registration fee, reduced with proof of Medicaid, SSDI, SSI or veteran status.

Good to know

  • Conditions include cancer with pain, nausea or wasting, seizures, MS with spasms, HIV/AIDS, Crohn's, UC, ALS, Parkinson's, chronic pain, PTSD, corticobasal degeneration, severe autism and terminal illness under one year.
  • The practitioner agrees to determine annually whether the patient still has the condition; cards last one year, and renewal requires recertification.
  • Before certifying, the practitioner gives the patient HHS's explanatory information on therapeutic use and the possible risks, benefits and side effects.
  • Purchases above 4.5 g THC per 90 days need the separate waiver, form 470-0192, from the same practitioner.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign Iowa's medical cannabis certification?

A health care practitioner licensed in Iowa: an MD or DO, a physician assistant or advanced practice registered nurse who is the patient's primary care provider, or a podiatrist. The form defines primary care provider as any practitioner involved in diagnosing and treating the patient's debilitating condition.

How long is an Iowa practitioner certification valid?

The Bureau of Cannabis Regulation must receive the certification within 60 days of the practitioner's signature date. The registration card that results is valid for one year, and renewing it requires recertification by a practitioner, the same forms and payment.

Does the certification alone authorize an Iowa medical cannabis card?

No. The attestation states that the certification does not by itself authorize a registration card. The patient must also submit the application, a copy of a driver's license or non-operator ID and the fee, and a designated caregiver files a separate application.
