Florida MMUR Physician Certification and Order

Florida patients can obtain medical marijuana only after a qualified physician certifies them and enters an order in the Medical Marijuana Use Registry (MMUR). The physician confirms a qualifying condition, performs an in-person exam and full history, finds the risks reasonable against the potential benefit, and records the order's route and amount. Each order covers no more than a 70-day supply.

Who completes it

A qualified physician, meaning a Florida MD or DO with an active, unrestricted license who completed the 2-hour FMA or FOMA course and exam, completes the whole electronic certification and order. Medical directors employed by a treatment center may not order.

Submitting and payment

How to submit it
Enter it in the Medical Marijuana Use Registry; update any change to the order within 7 days, and deactivate the patient's registration when treatment stops.

Good to know

  • The patient signs the approved consent form, DH-MQA-5026, each time the physician issues a certification.
  • The 2-hour course and exam must be repeated each time the physician renews their Florida license, and qualified physicians appear in OMMU's public physician search.
  • Qualifying conditions include cancer, epilepsy, glaucoma, HIV/AIDS, PTSD, ALS, Crohn's, Parkinson's, MS, comparable conditions, a terminal condition and chronic nonmalignant pain from a qualifying condition.
  • A terminal condition must be diagnosed by a physician other than the certifying physician.

No more than a 70-day supply per order. The order entered in the Medical Marijuana Use Registry must reflect the physician's order, including route and amount, and any change must be updated in the registry within 7 days.

Do Florida minors need a second physician for medical marijuana?
