Hawaii 329 Medical Cannabis Provider Certification

Hawaii's Medical Cannabis Registry issues a 329 card only after a certifying physician or APRN completes the provider step of the patient's application. The provider certifies a recognized debilitating condition, certifies as primary treating provider for another condition, or certifies a hospice patient, then submits the application to the Department of Health (DOH) for approval.

Who completes it

A Hawaii-licensed physician or APRN with a Hawaii controlled substance registration and a bona fide provider-patient relationship, approved in the registry, completes the provider certification. Patients or caregivers sign the separate 329 Section A or Section B certifications.

Submitting and payment

How to submit it
Patient-initiated applications arrive in the provider's pending queue in the Medical Cannabis Registry, where the provider certifies and submits to DOH; providers can also start provider-initiated applications for the patient.
Who pays
The patient pays the Department of Health registry fee.

Good to know

  • A new registry system launched on 9/30/2026; providers from the previous MedMJ system were migrated automatically and do not need to register again.
  • New providers set up an account through MyPVL and mail DOH a wet-signed 329 Electronic Signature Agreement before certifying electronically.
  • DOH updated its E-Certification Guide and Provider-Initiated Application Guide on 10/12/2026 and posts video walkthroughs for both processes.
  • To end a registration, the physician files the separate 329 Request to Void Patient's Registration Card.

Frequently asked questions

Do Hawaii providers need to re-register for the new 329 registry?

No. DOH says existing providers from the previous MedMJ system are migrated automatically into the new registry system that launched on 9/30/2026. New providers register using DOH's New Medical Provider Registration Guide and submit the Electronic Signature Agreement.

Can an APRN certify a Hawaii 329 medical cannabis patient?

Yes. Both Hawaii-licensed physicians and APRNs can certify, provided they hold a Hawaii controlled substance registration, have a bona fide provider-patient relationship with the patient, and have been approved in the registry with a signed Electronic Signature Agreement.

What is the difference between patient-initiated and provider-initiated 329 applications?

In a patient-initiated application, the patient applies and the application lands in the provider's pending queue for certification. In a provider-initiated application, the provider starts the application for the patient, following DOH's Provider-Initiated Application Guide and using the related 329 patient forms.
