North Dakota Medical Marijuana Written Certification

North Dakota makes a health care provider's written certification a mandatory part of every medical marijuana patient application. The patient starts the application in the program's registration system and enters the provider's name and email; the provider receives an email request, completes the certification online, and the system links it to the application for program staff to review before a registry ID card is issued.

Who completes it

A physician, physician assistant or APRN licensed in good standing in North Dakota completes the whole online certification, and only when a bona fide provider-patient relationship exists. The patient completes the rest of the application, including the provider's contact details.

Submitting and payment

How to submit it
Complete it inside the ND Medical Marijuana registration system after the emailed request arrives; the system attaches it to the patient's application, so there is nothing to print or mail.

Good to know

  • A bona fide relationship means you reviewed records, completed a full assessment, keep records, provide continued care for the qualifying condition and expect to follow up; it cannot exist solely for certification.
  • For a minor (under 19), document on the certification that you explained the risks and benefits of pediatric medical marijuana to the parent or guardian; minors cannot buy dried leaves and flower.
  • If you select cancer, you may authorize an enhanced amount of dried leaves and flower for an adult: up to 6 ounces in 30 days instead of the standard 2.5 ounces.
  • A provider with a financial interest in a compassion center may not issue a certification. Veterans receiving VA care and hospice patients can apply with other documents instead.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Why can't I see my patient's request in the ND registration system?

The program says this usually happens when the patient entered the provider's email address incorrectly, or started an application without fully completing and submitting it. Ask the patient to review the application and verify every detail; once it is submitted with the right email, the request appears on your pending list.

Can a provider rescind a North Dakota written certification?

Yes. A provider can rescind it after the card is issued, for example if the patient no longer has the qualifying condition or the bona fide relationship has ended. Contact the Medical Marijuana Program in writing; email suffices. Providers who retire or stop follow-up must notify the program, and their patients then have 90 days to renew with another provider.

Is a North Dakota written certification a prescription?

No. The program's provider overview says the written certification is not a prescription. It is a mandatory part of the patient's application, and state law protects providers from arrest, prosecution or loss of privileges for issuing one or for refusing to. No provider is required to provide a written certification.
