Connecticut Medical Marijuana Patient Certification

Connecticut patients cannot register for the Medical Marijuana Program until a certifying prescriber starts their application. The physician, physician assistant or APRN treating the patient for a qualifying condition certifies online that the patient meets the medical prerequisites for a registration certificate, after entering a patient profile with a valid email address and primary phone number.

Who completes it

A Connecticut-licensed physician, PA or APRN treating the patient for the qualifying condition completes the whole electronic certification, including the patient profile. The patient and any caregiver then complete their own registration.

Submitting and payment

How to submit it
Create a DAS Business Network account, log in to the online registration system with your DEA number and the last four digits of your Social Security number, and submit the certification there; there is no paper form.

Good to know

  • DCP strongly encourages prescribers to review its Physician Quick Reference Card before applying, and posts FAQs for prescribers on the same knowledge base.
  • The Board of Physicians has approved more than 40 adult conditions, including cancer, PTSD, sickle cell disease, Ehlers-Danlos syndrome with chronic pain and autism spectrum disorder.
  • Patients under 18 qualify only for a shorter list, such as cerebral palsy, cystic fibrosis, severe epilepsy, terminal illness, muscular dystrophy and intractable neuropathic pain.
  • To end a certification, the certifying physician files DCP's separate Physician Decertification Form, which gives the patient 30 days to obtain a new certification.
