Maryland Medical Cannabis Written Certification

Maryland patients need a written certification from a registered Certifying Provider to join the Maryland Cannabis Administration (MCA) registry. The provider confirms a bona fide provider-patient relationship, a severe qualifying condition for which other treatments have proven ineffective, and symptoms that can reasonably be expected to be relieved by medical cannabis, then issues the certification electronically.

Who completes it

A registered Certifying Provider completes the whole certification: a physician, PA, podiatrist, dentist, nurse practitioner or nurse-midwife with an active, unrestricted Maryland license and Maryland controlled substances registration.

Submitting and payment

How to submit it
Issue it electronically in the MCA registry through the provider's OneStop account, after registering with MCA as a Certifying Provider; the provider can later amend or revoke it.

Good to know

  • Qualifying conditions are cachexia, anorexia, wasting syndrome, severe pain, severe nausea, seizures, severe or persistent muscle spasms, glaucoma, PTSD and chronic pain.
  • The provider reviews relevant records, completes an annual assessment of history and current condition in person or by telehealth, and keeps standardized medical records.
  • Certifications may be amended or revoked on medical grounds, or if the patient meets exclusion criteria such as suspected cannabis abuse or diversion.
  • MCA's related forms include a HIPAA Authorization Form for minor patients and a Certifying Provider Inactivation Request Form.
