Oklahoma OMMA Physician Recommendation Form (Minor)

Oklahoma requires two physician recommendations before a patient under 18 can receive a medical marijuana license, and this two-page form holds both: page 1 for the first physician and page 2 for the second. The parent or legal guardian uploads it with the minor's online license application. Each page mirrors the adult form, with license term, optional ICD-10-CM conditions, caregiver certification and attestation.

Who completes it

Two Oklahoma MD, DO or podiatric physicians in good standing each complete and sign one page, the second within 30 days of the first. The parent or legal guardian submits the application, and a birth certificate is among the accepted proofs of the minor's identity.

Submitting and payment

How to submit it
Give both pages to the parent or legal guardian, who uploads them with the minor patient's online license application; the second recommendation must be dated within 30 days of the application's submission.

Good to know

  • The two recommendations must be dated within 30 days of each other, and the second within 30 days of submission, so timing has to be coordinated with the other physician.
  • Both physicians attest to a bona fide physician-patient relationship, an established medical record and a condition likely to benefit; the ICD-10-CM condition codes remain optional.
  • From January 1, 2026, each recommending physician must be registered with OMMA and complete the initial medical marijuana education course, with annual continuing education.
  • To end a minor's license later, both recommending physicians must complete the Physician Termination of Patient License Form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How many physicians must sign for a minor medical marijuana license in Oklahoma?

Two. OMMA's minor form has a first-physician page and a second page for another physician, and applications for minors must include recommendations from both. The two must be dated within 30 days of each other, and the second within 30 days of the application's submission date.

Can a physician certify that a minor needs a caregiver?

Yes. Each page has an optional caregiver section with a separate physician signature, certifying that the patient is homebound or cannot self-administer or purchase medical marijuana due to a developmental disability or a physical or cognitive impairment, and would benefit from a licensed caregiver.
