Oklahoma OMMA Physician Recommendation Form (Adult)

Oklahoma adults applying for a medical marijuana patient license upload this one-page recommendation with their online application in the OMMA MedPortal. The physician chooses a 2-year or 60-day temporary license, may list conditions by ICD-10-CM code, and can certify that the patient needs a caregiver. The application must be submitted within 30 days of the physician's signature.

Who completes it

An Oklahoma MD, DO or podiatric physician in good standing and registered with OMMA completes the whole form, including the patient's identity verification and the physician attestation. The patient submits it with their own online application.

Submitting and payment

How to submit it
Give the signed form to the patient, who uploads it with the online patient license application in the OMMA MedPortal within 30 days of the date you signed.

Good to know

  • Oklahoma has no qualifying-condition list, so the ICD-10-CM section is optional; the attestation covers a bona fide relationship, a medical record and likely therapeutic or palliative benefit.
  • The optional caregiver section needs its own physician signature certifying the patient is homebound or cannot self-administer or purchase due to a developmental disability or physical or cognitive impairment.
  • From January 1, 2026, physicians must register with OMMA and complete an initial medical marijuana education course, then renew continuing medical education annually.
  • Your office address must match the licensure board's record, and you may not practice at a dispensary's address, including virtual visits with patients located at a dispensary.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long is the Oklahoma physician recommendation valid?

The patient must submit the online license application within 30 days of the date the physician signs the form. On the form, the physician recommends either a 2-year license or a 60-day temporary license. The adult form posted on OMMA's forms page is the version updated 4.22.2022.

Who can sign an OMMA physician recommendation?

A doctor of medicine, doctor of osteopathic medicine or podiatric physician with a valid, unrestricted Oklahoma license, registered with OMMA and up to date on the required medical marijuana education. The licensing board, license number and NPI go in the physician section, and the physician attests to having verified the patient's identity.

Do I have to list a diagnosis on the Oklahoma form?

No. Oklahoma does not limit medical marijuana to listed conditions, so the medical conditions section, with space for three ICD-10-CM codes and descriptions, is optional. The required attestation states that you determined a condition for which the patient is likely to receive therapeutic or palliative benefit.
