OMMA Physician Termination of Patient License Form

Oklahoma's recommending physicians use this one-page form to tell OMMA that a patient no longer meets the requirements for medical marijuana. On successful submission, OMMA terminates the patient's license and any associated caregiver license, then notifies the patient and sends them a copy of the form. The patient is not refunded the license application fee.

Who completes it

The physician who signed the patient's recommendation form completes the patient information, physician information and attestation, and signs. For a patient under 18, both recommending physicians must complete the form.

Submitting and payment

How to submit it
Email it to OMMA's physician mailbox or mail it to OMMA, as the form instructs; OMMA verifies the details with the recommending physician before terminating the license.
Who pays
The patient is not refunded the license application fee when the license is terminated.

Good to know

  • Give complete, accurate contact details: OMMA must verify the form with the recommending physician, and if it cannot, the patient's license is not terminated.
  • The physician section asks for office address, email, phone number, medical license number and NPI, alongside the patient's name, date of birth and whether the patient is a minor.
  • Terminating a patient's license also terminates the associated caregiver license or licenses, and OMMA sends the patient a copy of the form with its notice.
  • OMMA's physicians page directs recommending physicians to this form when they withdraw a recommendation; the posted version was updated 4.21.2022.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

How do I terminate a patient's medical marijuana license in Oklahoma?

Complete OMMA's Physician Termination of Patient License Form, attesting that you signed the patient's recommendation and that the patient no longer meets the requirements, then email or mail it to OMMA. OMMA verifies the information with you; if it cannot, the license stays active. For minors, both recommending physicians must submit the form.

Does the patient get a refund when the license is terminated?

No. In the attestation, the physician acknowledges that submitting the form terminates the patient's license and that the patient will not be refunded the license application fee. Any caregiver license tied to the patient is terminated as well, and OMMA notifies the patient along with a copy of the form.
