NJ Medicinal Cannabis Provider Authorization

New Jersey's Medicinal Cannabis Program has no paper certification: a registered practitioner authorizes the patient in the Cannabis Regulatory Commission's provider portal, recording the qualifying condition and the monthly allotment. The patient then completes registration. The same portal lets the practitioner edit allotments, transfer in patients and manage staff access.

Who completes it

A New Jersey-licensed MD, DO, PA or NP with an active CDS registration and a physical practice in the state, registered with the CRC, completes the electronic authorization. The patient completes the registration that follows.

Submitting and payment

How to submit it
Enter the authorization in the CRC provider portal, after which the patient completes their own registration. You decide how often the patient needs a new authorization.
Who pays
Medicinal cannabis is not covered by any New Jersey health plan, though the CRC notes that recommendations can be discussed during a covered visit for the qualifying condition.

Good to know

  • A bona fide relationship requires ongoing care for the condition plus one of: a relationship of at least a year, four or more visits for it, or assumed management after a comprehensive history and exam.
  • Qualifying conditions include ALS, anxiety, cancer, chronic pain, dysmenorrhea, glaucoma, IBD, migraine, MS, opioid use disorder, PTSD, seizure disorders, Tourette syndrome and terminal illness with a prognosis under 12 months.
  • The Jake Honig Act caps dispensing at 3 ounces (84 grams) per 30 days, and you set the dose up to that limit; terminally ill patients have no limit.
  • Out-of-state practitioners cannot take part: the CRC requires active New Jersey credentials and a physical office in the state.

Frequently asked questions

Can New Jersey practitioners authorize medicinal cannabis by telehealth?

Yes. The CRC says practitioners may use telemedicine to evaluate the patient, make a diagnosis and manage treatment of the qualifying condition. The bona fide relationship rules still apply, and the practitioner must hold New Jersey credentials and keep a physical office in the state.

How do I change a patient's cannabis allotment in New Jersey?

Log in to the CRC provider portal and edit the patient's authorized allotment at any time. The maximum a patient may be dispensed is 3 ounces (84 grams) in a 30-day period, except for terminally ill patients, who have no limit. To take over a patient from another practice, register the patient with your practice using their registry ID.

What is required to authorize cannabis for a minor in New Jersey?

Explain the risks and benefits to the minor and the parent or legal custodian and document that discussion in the medical record. If you are not trained in pediatric care, obtain written confirmation from a pediatric-trained practitioner when you execute the authorization; there is no state form for that letter, and its author need not be registered.
