Arizona Medical Marijuana Physician Certification

Arizona issues a medical marijuana registry card only with a written certification on this department-provided form. The physician checks the debilitating condition under A.R.S. 36-2801, initials attestations covering the diagnosis, the medical record, a recent exam, record and PMP review, risk counseling, PTSD treatment, dispensary relationships and pregnancy risks, then attests the patient is likely to benefit.

Who completes it

An Arizona-licensed MD, DO, naturopathic physician (NMD/ND) or homeopathic physician (MD(H) or DO(H)) completes and signs the whole form, entering their license number and type.

Submitting and payment

How to submit it
Give the signed form to the patient, who uploads it with the online registry ID card application in the ADHS Individual Licensing Portal.

Good to know

  • The physical exam must fall within the last 90 calendar days, and the physician reviews 12 months of records from other treating physicians plus the Arizona controlled substances PMP profile.
  • If PTSD is checked, the physician also initials item 6, confirming evidence that the patient is currently in conventional PTSD treatment.
  • Item 8 confirms counseling on risks to fetuses and breastfed infants, and that use in pregnancy may lead to a report to the Department of Child Safety.
  • Patients under 18 also need a Reviewing Physician Certification from a second, different physician, uploaded with the same application.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
