Delaware Pediatric Medical Marijuana Certification

Patients aged 17 or younger in Delaware need this certification, on page 3 of the Pediatric Medical Marijuana Patient Application, before they can receive a pediatric registry card. A pediatric specialty practitioner identifies the medical condition(s) qualifying the child, and the parent or guardian submits it with the application, the attestations and the fee.

Who completes it

A Delaware-licensed pediatric neurologist, gastroenterologist, oncologist, palliative care specialist, psychiatrist or developmental pediatrician (MD, DO, APN or PA) completes the entire section and signs. The parent or guardian(s) complete and initial pages 1 and 2.

Submitting and payment

How to submit it
The parent or guardian mails the original with the pediatric application so the program receives it within 90 days of the practitioner's signature; faxed and electronic copies are not accepted.
Who pays
The parent or guardian pays one nonrefundable application fee, which includes the parent or guardian fees and varies with a one-, two- or three-year card.

Good to know

  • Signature stamps are not accepted, so the practitioner must sign the certification personally.
  • The instructions name four specialties, while the practitioner block also lists pediatric psychiatrists and developmental pediatricians, for six eligible specialties in all.
  • A second parent or guardian is listed only if a second caregiver card is needed, and both guardians initial all three attestation statements.
