Colorado DR 8560: Uniform Certification Form

Since January 1, 2022, under HB21-1317, Colorado medical marijuana patients who need to buy more than the statutory daily sales limit must have this Marijuana Enforcement Division (MED) form from their recommending provider. The provider certifies the authorized daily quantity, and the patient presents the form at medical marijuana stores. It is separate from the CDPHE registry certification and is not filed with it.

Who completes it

The patient's recommending provider, an MD or DO or an eligible advanced practice practitioner, completes the provider certification of the authorized daily quantity.

Submitting and payment

How to submit it
Give it to the patient, who presents it to medical marijuana stores; it is not submitted with the Medical Marijuana Registry application.

Good to know

  • MED posts a Spanish version, DR 8560SP, as a separate fillable PDF on its forms page next to the English form.
  • The current English fillable file is dated 02/07/2024 in MED's forms library.
  • The provider's separate CDPHE registry certification also records THC potency, directions for use, product form and any daily quantity above the limit.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is the Colorado Uniform Certification Form available in Spanish?

Yes. The Marijuana Enforcement Division publishes the Spanish version, Formulario de certificación uniforme (Marihuana Medicinal), as DR 8560SP alongside the English DR 8560 on its forms page. Both are fillable PDFs that the recommending provider completes for the patient.

Does DR 8560 get submitted to the Colorado registry?

No. The patient keeps the completed form and presents it to medical marijuana stores to buy above the daily sales limit. The registry ID card itself comes from the provider's separate online certification to CDPHE's Medical Marijuana Registry.

When is Colorado form DR 8560 required?

When a recommending provider authorizes a patient to purchase more than the statutory daily medical marijuana sales limit. The requirement has applied since January 1, 2022, under HB21-1317, and the form is used at the point of sale rather than at registration.
