Montana PSDMC-Minor Physician Statement for Minors

Minors applying for a Montana medical marijuana registry card need this four-page physician statement. In Parts A and B the treating or referral physician certifies the qualifying condition, describes it, the treatments used and activity restrictions, and sets the period of use. In Part C a second physician examines the minor and concurs, unless the first physician is an oncologist, neurologist or epileptologist.

Who completes it

A Montana-licensed MD or DO completes Parts A and B and signs; a second physician, after a physical exam and records review, initials and signs Part C. The parent or guardian completes the minor patient application.

Submitting and payment

How to submit it
Give the completed original to the family; the parent or guardian uploads it with the minor patient application in TAP within 60 days of the physician's assessment.

Good to know

  • Part C is required by 16-12-508(4), MCA; the second physician examines the minor independently of the treating physician and concurs with the diagnosis.
  • Part A asks the physician to mark whether they are a neurologist, oncologist or epileptologist, which removes the need for Part C.
  • The three Part B questions on the condition, treatments and activity restrictions must be answered on the form or in attached documentation.
  • The current form is V5 4/2026, and the resulting card expires no later than one year after issuance.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

When does a Montana minor need a second physician?

Always, unless the physician completing Part A is a licensed oncologist, neurologist or epileptologist. Otherwise a second physician must perform a physical exam, review the minor's records kept by the treating or referral physician, and sign Part C concurring with the diagnosis.

What does the second physician certify on Montana's PSDMC-Minor?

The second physician initials three statements: they reviewed the minor's medical records as maintained by the treating or referral physician, the potential benefits likely outweigh the health risks, and the information is true and correct. They then sign, concurring with the diagnosis after an independent exam.
