Minnesota Medical Cannabis Patient Certification

Minnesota residents enroll in the medical cannabis program only after a registered health care practitioner certifies a qualifying medical condition in the Medical Cannabis Registry. The practitioner attests to managing the condition on an ongoing basis, after examining the patient, reviewing records and developing an ongoing treatment plan. The certification triggers an email inviting the patient to complete enrollment.

Who completes it

A Minnesota-licensed physician, PA or APRN with primary responsibility for the patient's qualifying condition, registered in the Medical Cannabis Registry, completes the whole electronic certification.

Submitting and payment

How to submit it
Log in to the Medical Cannabis Registry and select Certify Patient; the patient then receives an email to complete enrollment.

Good to know

  • Recertification and re-enrollment are due every three years, and practitioners can recertify up to 90 days before the certification expires.
  • The program moved from the Minnesota Department of Health to the Office of Cannabis Management's Division of Medical Cannabis in 2025.
  • Practitioners first create a registry account with the Health Care Practitioner Registration Form before they can certify any patient.
