Oregon Attending Provider Statement (OHA le9265)

Oregon Medical Marijuana Program applicants need their attending provider to attest that they have a debilitating medical condition and that medical marijuana may mitigate its symptoms or effects. The patient submits this statement with the OMMP application (le9240) within 90 days of the provider's signature. Instead of the statement, the provider may give the patient relevant portions of the medical record containing the same information.

Who completes it

An Oregon attending provider with primary responsibility for the patient's care, as primary care provider, specialist or consultant, signs the whole form: an MD or DO, physician associate, nurse practitioner, clinical nurse specialist, CRNA or naturopathic physician. The patient completes the OMMP application.

Submitting and payment

How to submit it
Give the signed statement to the patient, who submits it with the OMMP application within 90 days of your signature; for renewals, it must be signed within the 90 days before the card expires.

Good to know

  • Before signing, the provider must have reviewed the patient's records at the patient's request, done a thorough physical examination, provided or planned follow-up care, and documented these in the chart.
  • Qualifying conditions include cancer, glaucoma, degenerative or pervasive neurological conditions, HIV/AIDS, PTSD, and conditions or treatments causing cachexia, severe pain, severe nausea, seizures or persistent muscle spasms.
  • Provider protections are in ORS 475C.891, 475C.892 and 475C.916; OHA directs questions about how a licensing board applies them to that board.
  • A related companion, the property owner consent form, must be notarized for registry cards issued from January 1, 2026.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

How long is the Oregon Attending Provider Statement valid?

First-time applicants must submit the statement, or the relevant medical records, within 90 days of the attending provider's signature. For renewals, application materials are due within 90 days of the card's expiration date, and the statement must be signed within that same 90-day period.

Can a naturopath or nurse practitioner sign the Oregon APS?

Yes. Oregon's list of attending providers includes MDs and DOs, physician associates, nurse practitioners, clinical nurse specialists, certified registered nurse anesthetists and naturopathic physicians. The provider must have primary responsibility for the patient's care, as the primary care provider, a specialist, or a consultant asked to treat the patient.

Can I send medical records instead of the Attending Provider Statement?

Yes. OHA allows the attending provider to give the patient relevant portions of the medical record instead, as long as they contain the same information: a diagnosed debilitating medical condition and a statement that medical marijuana may mitigate its symptoms or effects. The same 90-day submission window applies.
