Kentucky Medical Cannabis Practitioner Certification

Kentucky's Medical Cannabis Program, launched January 1, 2025 under KRS 218B, requires an electronic written certification from an authorized practitioner before a patient can apply for a registry card. After an in-person exam, the practitioner records that the patient has a qualifying condition and is likely to benefit safely and effectively, then enters the certification in the state portal.

Who completes it

A Kentucky physician authorized by the Board of Medical Licensure (KBML) or an APRN authorized by the Board of Nursing (KBN), registered with the program, completes the whole electronic certification. The patient then applies online.

Submitting and payment

How to submit it
In the Medical Cannabis Practitioner Registration Portal, choose Create New Application, Show All, then Electronic Practitioner Certification, and record it within 24 hours of the in-person exam.

Good to know

  • Before certifying, review at least 12 months of the patient's KASPER report, discuss risks, side effects and drug interactions, and set an expectation of follow-up care.
  • For minors, the risk discussion is held with the custodial parent or legal guardian, whose consent is required before certifying.
  • Statutory conditions are cancer, chronic severe or intractable pain, epilepsy or intractable seizures, MS or spasticity, chronic nausea or cyclical vomiting, and PTSD.
  • Physician authorization requires 6 hours of continuing education; pages 31 to 40 of the Authorized Practitioner Registration Guide walk through the certification steps.
