Clinician certifications for disabled hunter and angler permits

State wildlife agency forms where a clinician certifies a disability so a patient can use a crossbow, hunt from a vehicle, or get a free or reduced-fee license.

State wildlife agencies let hunters and anglers with disabilities use adapted methods or get free or reduced-fee licenses, and many of these permits rest on a clinician's certification. The common types are crossbow or modified bow permits, such as California's DFW 537 and Michigan's PR9134; permits to shoot from a stationary vehicle, such as Pennsylvania's PGC-707-LE and New York's Non-Ambulatory Hunter Permit; and free fishing licenses for blindness, mobility impairment or developmental disability, such as California's DFW355B.

Check who may sign. Many forms accept only a licensed physician, but Idaho, Iowa, Oregon and Wyoming also accept PAs and nurse practitioners, Michigan accepts physical and occupational therapists, and some states take chiropractors. Vision permits often go to an optometrist or ophthalmologist. Illinois requires a witness to the physician's signature. In Kentucky, and for Missouri's fishing exemption, the completed form is itself the permit and stays with the hunter or angler. Vero Documents fills PDF forms from the visit note and the chart; the clinician reviews and signs.

Frequently asked questions

Who signs the disabled archer permit in California?

A physician signs the DFW 537 Disabled Archer Permit Application, certifying a disability that prevents drawing and holding a bow, and records their license number. The hunter can then use a crossbow or a bow-locking device during archery seasons. The application is mailed to the CDFW License and Revenue Branch, carries no fee, and the permit is valid for one license year, July 1 to June 30. It must be carried while hunting.

What does the Pennsylvania PGC-707-LE require from a physician?

The physician certifies a qualifying mobility, lung or cardiovascular disability so a licensed hunter can get a Disabled Person Permit to shoot from a stationary vehicle. Chiropractors may certify mobility only, not lung or cardiovascular disease. The applicant completes Section I only. The Game Commission may contact the physician, and a refusal to discuss the case returns the application. A handicapped parking permit does not automatically qualify.

Can a nurse practitioner sign a disabled hunting permit?

On some state forms. Idaho's disability certification and BA-116, Iowa's severely disabled deer license application, Oregon's Disabilities Hunting and Fishing Permit, Washington's Hunter / Fisher Disability Status Application and Wyoming's Disabled Hunter Permit accept NPs or ARNPs, and Montana accepts APRNs. Louisiana's Physically Challenged Hunters Permit needs a Louisiana-licensed physician, not an NP or PA, and California's FG355D also excludes NPs and PAs.

Who certifies blindness for a free or lifetime fishing license?

It varies by state. California's DFW355B is certified by an optometrist or ophthalmologist for legal blindness: acuity of 20/200 or a field of 20 degrees or less. Missouri's fishing permit exemption takes an ophthalmologist or optometrist for vision. Montana's lifetime fishing license for the blind is certified by a physician. Vermont's legally blind license has no clinician form and accepts any physician document stating legal blindness.
