Washington WDFW Hunter/Fisher Disability Status Form

Washington's Department of Fish and Wildlife grants Disability Status to hunters and fishers with a permanent inoperable disability, opening access to a companion card, designated harvester, reduced-fee licenses and Special Use Permits. On this two-page application, the clinician signs beside each qualifying category. Page 2 prints the WAC 220-200-160 definitions the clinician certifies against.

Who completes it

The applicant completes and signs the information section. A licensed MD, ARNP or PA with knowledge and documentation of the disability signs beside each qualifying category, then signs the certification with name, title, address, phone and medical license number or NPI.

Frequently asked questions

A licensed MD, ARNP or PA. The form defines physician to include a medical doctor, a physician assistant, an osteopathic physician assistant and an advanced registered nurse practitioner. The clinician must have knowledge and documentation of the disability, and the license number or NPI is mandatory.
