Vermont Paraplegic Mobility Disability License Form

Vermont may issue a resident with paraplegia or a permanent, severe physical mobility disability a free permanent fishing license or, if qualified, a free permanent hunting and fishing combination license. The 2026 packet includes the physician's Statement of Disability, the sporting license application and a license affidavit. Nonresidents from reciprocal states can get a free one-year license.

Who completes it

The applicant fills in their details on the Statement of Disability, signs to authorize the state to contact the physician, and completes the sporting license application. The physician checks paraplegia or a permanent severe mobility disability, confirms the applicant is under their care, and signs with name, address and phone.

Submitting and payment

How to submit it
The applicant mails the documents to Vermont Fish & Wildlife in Montpelier or emails them to the licensing office. Incomplete applications are rejected until complete.
Who pays
The license is free; documentation is submitted in place of a fee, and no fee is set for the physician's statement.

Good to know

  • Paraplegia is defined by statute as permanent paralysis of the lower half of the body involving both legs; the alternative is a permanent mobility disability that prevents safely navigating the woods on foot.
  • Hunting privileges need proof of a previous hunting license or hunter education certificate; applicants who cannot find an old license can use the included license affidavit.
  • Vermont shares reciprocity with New Hampshire and Maine; residents of those states may submit their own state's mobility disability license instead of the physician statement.
  • Vermont reserves the right to contact the physician to verify eligibility. Legal blindness uses a separate application that accepts a physician's document instead of this form.

Frequently asked questions

What does the physician certify on Vermont's mobility disability license form?

That the applicant is currently under their professional care and has either paraplegia, meaning permanent paralysis of the lower half of the body involving both legs, or a permanent severe mobility disability that prevents safely navigating through the woods on foot. The physician checks one box and signs.

Can nonresidents get Vermont's mobility disability license?

Yes, if they live in a state that gives Vermont residents a reciprocal privilege. They can receive a free one-year fishing or combination license instead of a permanent one. Vermont shares reciprocity with New Hampshire and Maine, whose residents may submit their home state's license instead.
