Vermont Off-Road Hunting Permit Statement of Disability

Vermont Fish & Wildlife can issue a licensed hunter a permit to hunt from a motor vehicle off the right of way if the hunter is paraplegic or certified by a physician as unable to pursue game because of a permanent severe physical disability. This one-page statement is that certification. The permit is issued to permanently disabled people only.

Who completes it

The hunter fills in their details and signs, authorizing the state to contact the physician. A physician who has the hunter under professional care states the date care began, gives the medical opinion that the hunter cannot walk 25 feet from a roadway to pursue game, and signs with name, address and phone.

Submitting and payment

How to submit it
The statement is mailed or faxed to Vermont Fish & Wildlife in Montpelier. A Word version is also posted on the department's printable applications page.
Who pays
The statement lists no fee for the permit or for the physician's certification.

Good to know

  • The form asks physicians not to complete it if the disability is not permanent, if they are unsure it is permanent, or if the applicant can walk 25 feet.
  • Vermont reserves the right to call and verify the disability, and the hunter's signature authorizes the state to contact the physician.
  • The physician must state the date since which the applicant has been under their professional care.
  • Hunters with paraplegia or a severe mobility disability may also qualify for Vermont's free permanent mobility disability license, which uses a separate statement.
