Virginia DWR Disabled Hunter Treestand Exemption Form

Some Virginia localities require hunters to use an elevated treestand during certain seasons. Under Code of Virginia section 29.1-528.2, a hunter with a qualifying permanent physical disability is exempt, and this one-page DWR form is the physician's certification of that disability. The hunter carries it while hunting; it is not filed with the department.

Who completes it

The hunter completes the hunter information section and signs. A physician completes the Physician's Statement before the hunter goes afield, certifying the disability meets section 29.1-528.2, and adds the examination date, medical license number, signature, printed name and office contact details.

Submitting and payment

How to submit it
The completed form stays in the hunter's possession while hunting; it is not sent to DWR or to the county.
Who pays
The form lists no fee; it is kept by the hunter rather than filed with DWR.

  • Hunters who hold a Virginia permanently disabled or veteran disabled license do not need this form.
  • The form is not a hunting license or privilege and does not allow hunting or shooting from a vehicle. The current version is dated April 1, 2021.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
