Vermont Developmental Disability Fishing License Form

Vermont may issue a free permanent fishing license to a resident with a developmental disability. The three-page packet includes a cover letter, the Statement of Disability that the applicant's health care provider completes, and the Vermont sporting license application. The provider certifies that the applicant is under their care and meets the definition in 18 V.S.A. section 9302.

Who completes it

The applicant fills in their details on the Statement of Disability and signs, authorizing the state to contact the provider, and completes the sporting license application. The health care provider certifies eligibility and adds a printed name, address, phone, date and signature.

Submitting and payment

How to submit it
The applicant mails the statement and sporting license application to Vermont Fish & Wildlife in Montpelier, or emails them to the department's licensing office. Incomplete applications are rejected until complete.
Who pays
The license is free; the required documentation is submitted in place of a fee, and no fee is set for the provider's statement.
