Nevada Severely Disabled or Blind Combo License Form

Nevada residents with a severe physical disability or blindness can buy a reduced-fee specialty combination hunting and fishing license from the Nevada Department of Wildlife. This two-page application carries the resident's details and proof requirements on page 1 and a short physician statement on page 2. The license year runs one year from the date of purchase.

Who completes it

The applicant completes page 1, including Social Security number, residency details and signature; a parent signs for youth 17 and under. A physician, optometrist or ophthalmologist completes the page 2 physician statement, answering two yes/no questions, then signs, dates and prints their name and address.

Submitting and payment

How to submit it
The applicant returns the form with proof of residency to the nearest NDOW regional license office in Reno, Fallon, Elko or Las Vegas.
Who pays
The applicant pays NDOW the reduced specialty license fee plus processing fees; the form sets no fee for the physician statement.

Good to know

  • A severe physical disability means one that materially limits the person's ability to engage in gainful employment; the physician answers whether the permanent disability does so.
  • Blind means acuity no better than 20/200 in the better eye with correcting lenses, or a visual field in the better eye of 20 degrees or less.
  • Residents must have lived in Nevada for the six months before applying and attach dated ID showing residency; a previous year's license is not accepted as proof.
  • Hunting applicants born after January 1, 1960 must attach a hunter education certificate or a prior license showing course completion.
