Missouri Hunting Method Exemption Physician Statement

Missouri lets a properly licensed hunter with a qualifying disability hunt from a stationary vehicle if a licensed physician signs this statement of eligibility. The physician checks the conditions that apply, such as blindness or being unable to walk 50 feet without resting, and chooses a duration from 6 months to 5 years, since permanent status is no longer offered.

Who completes it

The applicant completes Section A and signs the detachable carry card. The physician completes Sections B and C, including license number and state, the probable duration, and signatures on both the form and the carry card.

Submitting and payment

How to submit it
The hunter mails the top portion to MDC's Protection Division within 10 days of the physician's signature and keeps the bottom card while hunting.

Good to know

  • Qualifying conditions include blindness, inability to walk 50 feet without resting, needing a walking aid or another person, FEV1 under one liter or PaO2 under 60, portable oxygen, and AHA class III or IV.
  • Missing information or failing to mail the top portion within 10 days invalidates the exemption. Age is not a factor in determining disability.
  • A new form is needed when the chosen duration ends. Shooting from or across a public road remains prohibited.
  • Wildlife violations in the past five years may make a hunter ineligible. Current form revised 3/2019; it asks for the applicant's SSN.

Frequently asked questions

How long does a Missouri hunting method exemption last?

The physician chooses a probable duration of 6 months, 1, 2, 3 or 5 years. Permanent status is no longer available, so the hunter submits a new form, with a new physician statement, when the chosen period expires.

What does the physician sign on Missouri's hunting method exemption?

The physician checks every qualifying condition in Section B, completes Section C with name, license number, state of license and facility details, selects the duration, and signs and dates it. The physician also signs and enters the date of issue on the detachable carry card.

Does the hunter have to send anything to MDC?

Yes. The top portion of the form must be mailed to the Missouri Department of Conservation's Protection Division within 10 days after the physician completes it, or the exemption is invalid. The hunter keeps the bottom card and shows it to any agent of the department on request.
