Missouri MDC Fishing Permit Exemption Statement

Missouri residents who are legally blind, permanently unable to move freely without a wheelchair, or so severely affected by cerebral palsy or a mental disorder that they cannot fish alone may fish without a permit. This statement of eligibility, signed by a physician, is the proof, and the angler carries it while fishing instead of sending it to the Missouri Department of Conservation.

Who completes it

The patient or guardian completes and signs Section A. The physician (or an ophthalmologist or optometrist for vision) checks the qualifying exemption in Section B and completes Section C with license number, state of license and signature.

Submitting and payment

How to submit it
The signed statement is kept with the angler while fishing and shown to any officer enforcing wildlife rules on request; it is not sent to MDC.

Frequently asked questions

Who can sign Missouri's fishing permit exemption?

A licensed physician for any of the three exemptions. For the vision exemption, a licensed ophthalmologist or optometrist may also sign. For cerebral palsy or a mental disorder, the rule calls for a physician qualified to evaluate and treat people with developmental disabilities.

Does the Missouri fishing exemption statement get mailed to MDC?

No. The form says to keep the signed statement while fishing and not send it to the Missouri Department of Conservation. The angler shows it to any officer enforcing wildlife rules, who may also inspect fish in possession.

Does the exemption cover trout fishing in Missouri?

Not fully. The exemption lets qualifying residents take fish, live bait and other listed species without a fishing permit, but trout permits or daily tags are still required where they are prescribed, according to the regulation printed on the form.
