Kentucky Mobility Impaired Access Permit (KDFWR)

Kentucky hunters and anglers with a severe, permanent mobility impairment can get a permit to use an ATV, electric wheelchair or other vehicle on designated tracts of certain wildlife management areas, and to shoot from a motionless vehicle with the engine off. A Kentucky-licensed physician attests to one or more of seven criteria, and the completed application serves as the permit.

Who completes it

The applicant fills in name, contact details and an ID number, then signs the terms. A physician licensed in Kentucky checks the qualifying impairment (a to g) and signs with printed name and business address.

Submitting and payment

How to submit it
The completed, signed application becomes the permit, valid for one license year (March 1 to the last day of February), and must be carried at all times while using it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign Kentucky's Mobility Impaired Access Permit?

A physician licensed in Kentucky. The physician attests that the applicant has one or more of the listed severe mobility impairments, such as permanent paralysis of a leg, a foot amputation, reliance on a wheelchair, crutches or walker, severe lung disease, portable oxygen, or an AHA class 3 or 4 cardiac condition.

How long is the Kentucky Mobility Impaired Access Permit valid?

No longer than one license year, which runs from March 1 to the last day of February. Because the completed application is the permit, a new signed application with a new physician attestation is needed for each license year.

What does the Mobility Impaired Access Permit allow?

Shooting from a motor vehicle or watercraft that is motionless with the engine off, operating an electric wheelchair or ATV on gated or ungated roads of designated WMA tracts where signs allow, and designating one qualified assistant. It does not replace any required license or permit.
