Kentucky Hunting Methods Exemption: Crossbow Permit

Kentucky lets a hunter who cannot use conventional archery equipment because of a disability use a crossbow during archery seasons, including bow-only deer season, with a hunting methods exemption. A licensed physician attests to the reason and states whether the disability is temporary or permanent, with a length of time, and the signed form itself becomes the permit.

Who completes it

The hunter fills in name, contact details and ID number, then signs the terms. A licensed physician completes the attestation, describing the disability, marking temporary or permanent with the length of time, and signing with business address.

Submitting and payment

How to submit it
The form is not returned to KDFWR: once completed and signed it is the hunter's permit and must be carried while hunting.

Good to know

  • The length of time is required. If the disability is temporary, the hunter must return to conventional archery equipment when the stated period ends.
  • KDFWR keeps no copies of the permit, so the hunter is responsible for keeping the signed form.
  • The hunter still needs the appropriate Kentucky hunting licenses and tags, and the crossbow must meet Kentucky's equipment regulations.
  • The one-page flat PDF asks the hunter for a Social Security or driver's license number as the ID.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What does the physician attest on Kentucky's crossbow permit?

That the named hunter is not able to use conventional archery equipment and must use a crossbow, followed by a description of the disability. The physician marks it temporary or permanent, gives the length of time, and signs with printed name and business address.

Does the Kentucky crossbow exemption get mailed to KDFWR?

No. The form says not to return it to KDFWR. Once the physician and hunter sign it, the completed form is the Hunting Methods Exemption Permit, and the hunter carries it with their license and permits while hunting.

How long does a Kentucky crossbow exemption last?

As long as the physician states. A permanent disability is marked permanent; for a temporary disability the physician gives a length of time, and the hunter must return to conventional hunting methods when that period ends. KDFWR requires the length of time on every form.
